Job summary Are you a nurse leader who has experience of working withing respiratory medicine who is passionate about providing outstanding care, if so, then we want to hear from you Respiratory Hospital at Home is designed to facilitate early/supported discharge of patients from acute medical beds and to prevent hospital admissions of patients with unstable respiratory disease in the community, so creating a 'virtual ward'.You provide a high standard of complex, enhanced care for patients, using enhanced levels of clinical judgement, skills and knowledge to avoid admission to hospital or support an early discharge. At times you will be part of an exciting pilot working within a Care Co-ordination hub, working alongside system partners to include SWASFT, 111, Rapid Response, Somerset Primary Link and District nursing teams taking referrals for Hospital at Home to ensure the patient is seen by the right team, in the right place, the first time. Main duties of the job Senior Respiratory Specialist Practitioners work as part of a multi-disciplinary clinical team across a wide range of settings, including hospitals, individual's homes, care homes and general practices. You will work at an enhanced level of practice with specific knowledge and skills in a field of expertise which, for these posts, is respiratory medicine. They manage a discrete aspect of a patient's care within their current level of practice, which will be particular to a specific context. You will provide a high standard of complex, enhanced care for patients, using enhanced levels of clinical judgement, skills and knowledge. In this instance we are looking for staff who will be able to support patients with respiratory illness in their home environment. You will work alongside the Clinical Lead's, to lead and manage a team of highly skilled practitioners. You will co-ordinate and manage shifts ensuring patient and staff safety is prioritised, ensuring referrals to the service are clinically appropriate and safe and will work alongside the care- co-ordination hub in supporting patients to receive the right care, in the right place by the right team the first time.You will lead and manage members of the clinical team ensuring staff are working to their clinical competence with areas of progression highlighted, challenging any behaviours or practice not aligned with the Trust Values. About us Respiratory Hospital Home is a county wide service providing care for patients whom have a Somerset GP. We have 4 community bases located at Harrison House, Bridgwater Hospital, West Mendip Hospital and Magnolia House, whilst supporting the two acute sites of Musgrove Park Hospital (MPH)and Yeovil District Hospital. This post is being advertised to cover the West side of the county, which includes the Taunton/Bridgwater area alongside MPH. We do cross cover within the service and you will be expected to support the South side of the county when patient need arises, this would be on a weekly basis.
