Key Duties and Responsibilities include: The PCN Clinical Pharmacist responsibilities include but are not limited to the following: To be a prescriber, or completing training to become prescribers and work with and alongside the general practice team To undertake the care management of patients with chronic diseases and to undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially older people, people in care homes, those with multiple co-morbidities (in particular frailty, COPD and asthma) and people with learning disabilities or autism (through STOMP Stop Over Medicaoin Programme) Provide expertise in the use of medicines and person-centred medicines optimisation across the network, including taking a central role in the clinical aspects of shared care protocols and liaison with specialist pharmacists to reduce inappropriate use of high-risk medicines Support the integration of general practice with wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, reduce waste, and promote self-care, provide better access to healthcare, and manage general practice workload. Managing Safety and CAS alerts Assisting with QOF and incentive scheme audits, PINCER safety indicators Attending Medicines Management Prescribing forums quarterly and assisting practices in achieving Prescribing incentive Scheme targets. CQC preparedness e.g. ensuring all prescribing protocols are up to date Care home support e.g. annual meds review of patient Discharge summary reconciliation Poly pharmacy Medication reviews Minor ailment clinic Medication review clinic: Respiratory, Diabetes, Cardiovascular Colostomy products etc Supervision and training of reception staff in issuing of repeat prescriptions and queries Supervision and training for new prescribing staff including trainees and nurse prescribers Attending Clinical and PCN meetings as required Telephone advice to patients for queries on medication and minor illness, self-care, sign posting to other services as needed. Review of repeat prescription policies and support staff in implementation. Key Collaborative Working Relationships PCN Pharmacy Team, including Clinical Pharmacists and Pharmacy Technicians working within the PCN/across practices PCN Clinical Directors, PCN Manager and Lead GPs GPs, nurses and other practice staff Other healthcare professionals involved in medicines management team including the ICB Medicines management team. Practice based GP prescribing Lead(s). Community nurses and other allied health professionals including dietetics. Community pharmacists and support staff Hospital staff with responsibilities for prescribing and medicines optimisation Care Home Staff The voluntary sector. Receiving referrals from Locality or Practice based Social Prescribers. The Multi-Disciplinary Team Education, learning and development It is the responsibility of the employee to comply with all organisational and statutory requirements (eg. health and safety, infection control, equality and diversity, confidentiality, safeguarding adults and children, information governance). Engages in annual appraisal, developing objectives to inform a Personal Development Plan, which may include 360-degree appraisal and use of patient feedback. Participate in teaching and training of medical, nursing, and all other practice staff. Supports the practice staff and responds to requests for advice and assistance. Complete all mandatory and statutory training required by the role. Takes responsibility for personal development, learning and performance and maintain education through attendance on any courses and/or study days necessary to ensure that professional development requirements are met. Undertakes additional training where necessary to provide enhanced services and participate in training programmes implemented by the PCN/practices as part of this employment. Understands and demonstrates the characteristics of a role model to members in the team and/or service. Demonstrates an understanding of current educational policies relevant to working areas of practice and keeps up to date with relevant clinical practice. Quality Under supervision and support of GPs in the practice, the post-holder will strive to maintain quality within the practices, and will: Participate in clinical governance activity and contribute to the improvement in quality of health outcomes through audit, risk management and Quality Improvement Alerts other team members to concerns about risk, quality, and safety Participates in investigation of incidents and events as required Identifies, applies, and disseminates research findings relating to own practice Collects data for audit purposes and uses clinical audit to monitor quality in the service Contributes to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance Works effectively with individuals in other agencies to meet patients needs Effectively manages own time, workload, and resources Meets timescales/deadlines for audits and written returns to ensure that the Practice meets quality standards and receives the designated funding.
