We are looking for a fixed-term Care Coordinator (particularly with experience in, and able to support with, safeguarding).
As a Safeguarding Care Co-ordinator, you will be supporting all key safeguarding activity across the practice; supporting the safeguarding leads and associated practices by co-ordinating activity and providing efficient, well-organised administrative and operational support to the clinicians and managers in the network to ensure effective, timely delivery of practice objectives.
Depending on your number of working hours, you may be expected to help out in other areas of the practice as well - for example, coordination of QOF areas or certain targeted groups of patients.

Main duties of the job
* Processing correspondence from external agencies pertaining to safeguarding.
* Maintaining practice safeguarding registers.
* Sharing relevant health information with other agencies when required as part of safeguarding processes.
* Supporting & monitoring training of practice staff.
* Supporting audit of safeguarding activities.
* Embed local & national learning from safeguarding reviews.
* Maintain an up-to-date log of contacts/services within the safeguarding arena.
* Maintain training/competence.
* Coding/summarising incoming correspondence from outside agencies.
* Gathering information to inform the assessment of risk when a concern is raised by a member of staff or advised to the practice by a partner agency.
* Reviewing ED attendances and flagging concerns to safeguarding leads.
* Escalating concerns to safeguarding leads where information received might suggest escalating risk.

Job responsibilities
Further to the Main Duties, these are the key aspects of the job description:
Maintaining practice safeguarding registers
* Review/manage safeguarding registers according to information received.
* Ensuring ongoing review of safeguarding registers to downgrade concerns & ensure registers & safeguarding flags are current.
* Summarising historic safeguarding concerns for new patient registrations.
* Ensuring safeguarding information is shared regarding patients deducted from practice list when they register elsewhere.
Sharing relevant health information with other agencies when required as part of safeguarding processes
* Pulling together appropriate information to feed into:
* Child protection Case Conferences
* Safeguarding Adults processes
* LAC health reports
* Team around the Adult meetings
* Statutory Safeguarding Reviews (e.g. SCR, SAR, DVHR)
* Rapid Reviews
* Organising MDT meetings to facilitate information sharing between GP safeguarding leads, health visitors, school nurses & midwives.
* Preparing agendas for MDTs.
* Update child & family records following MDTs.
Supporting & monitoring training of practice staff
* Ensure safeguarding training of staff is maintained to levels required within the intercollegiate documents.
* Assist with provision of training for non-clinical staff.
* Assist with preparation of training for clinical staff.
Supporting audit of safeguarding activities
* Process annual safeguarding assurance audit required by ICB.
* Undertake audits as required to assure practice/local safeguarding policies/processes are followed & to inform change as required.
Embed local & national learning from safeguarding reviews
* Work with ICB safeguarding team to ensure that learning from statutory reviews is shared and implemented.
* Complete & return action plans as required by ICB safeguarding team to demonstrate progress with implementation.
Maintain training / competence
* Attend ICB safeguarding leads briefings.
* Keep up to date with safeguarding relevant legislation.
Maintain an up-to-date log of contacts/services within the safeguarding arena
* Curate a directory of service/contacts to be accessible within the practice.

Person Specification
Qualifications
* Achieved grade C or above, in English and Maths GCSE or equivalent.
* NVQ Level III (Health and Social Care)
Experience
* Previous experience working with the general public.
* Experience of working autonomously and part of a team.
* Experience of safeguarding policies and procedures within a primary care context.
* Experience of working in Primary Care.
* Evidence of working within a multidisciplinary team.
* Experience of working without direct supervision.
Knowledge and Skills
* Ability to recognise and respond appropriately to risk and safeguarding concerns.
* Knowledge around importance of confidentiality and data protection.
* Good communication and interpersonal skills, including an ability to build rapport and establish good one-to-one relationships.
* Ability to deal with challenging behaviour and difficult conversations.
* Be able to offer support in a person-centred and non-judgmental way.
* Ability to effectively manage a variable workload.
* Ability to maintain accurate and concise records.
* Ability to provide information effectively.
