About the Role:
As a Reablement Worker you will work closely with our Therapy and Support Leads, Registered Manager, and other professionals to ensure a high level of person-centred care is delivered using a therapy led, individualised reablement care and support plan and risk assessments.
You will be responsible for facilitating admissions to the service, which will include liaising with families and other agencies to ensure a smooth transition to Borrowdale House. APPLICANTS WILL need to be medically certified as they will be administering medication.
Administration of medication according to policies and procedures, low level health care tasks and implementation of therapy guidance will be required, ensuring best practices are always applied and in the best interests of the person you are supporting.
The role will help challenge the status quo and help lead change to ensure Adults within South Tyneside are supported to Live Better Lives.
About You:
You will have experience of working in a health and social care environment, as a reablement or support worker. With excellent coordination and communication skills, be able to deliver personalised outcome based reablement plans and be able to develop and maintain effective relationships with a wide range of stakeholders.
You will need the confidence and ability to demonstrate sound professional judgements, when making decisions and have the ability to support real change.
