Clinical Nurse Specialist, Homeless Care Team
Band 6
As University Hospitals Dorset, we are a three-site acute trust with ‘university hospital’ status through our partnership with Bournemouth University, working with an annual budget of £730m.
We offer highly regarded health services, employing some 10,000 staff. Our ambitious £250m estates programme is well on the way to delivering state-of-the-art facilities over the next two years to meet the current and future healthcare needs of the populations we serve.
In an Area of Outstanding Natural Beauty with over seven miles of sandy beaches, you’ll find inspirational opportunities to achieve your perfect work/life balance. With the New Forest and the Purbecks on our doorstep, we’re also less than three hours from London by road or train.
We believe that having a diverse workforce and allowing people to be themselves is essential in ensuring we offer an inclusive, welcoming, and productive place to work.
We especially welcome applications from minority groups in the community who may be underrepresented in our workforce.
Job overview
An exciting opportunity has arisen to join an existing UHD multidisciplinary team, working on the Poole and Bournemouth sites, but also undertaking outreach to review patients in the community.
The aim of the service is to care for patients accessing UHD services who are homeless or at risk of homelessness and who may also have mental health, alcohol, and drug misuse issues. The role will also involve the care of patients undergoing treatment for Chronic Hepatitis C across Dorset as part of the Dorset Hepatitis C Elimination Programme.
This will involve working with partners in the community and on a mobile clinical van.
The service aims to enable patients to receive the best care that can be provided through improving staff education, acute care delivery, and pathways to community services.
You will be someone who enjoys the challenges this type of work represents and has a passion for caring for this patient group. Someone who is used to multi-agency, multi-disciplinary working in partnership with many agencies and organizations and is confident to see patients in hospital at the bedside, at the front door in the emergency and acute admission departments, as well as in the community.
