Job summary An exciting opportunity has arisen for a Major Trauma Rehabilitation Coordinator, within our Major Trauma Service at the Cambridge Major Trauma Centre. We are looking for a passionate and experienced physiotherapist with a background in musculoskeletal rehabilitation to facilitate and coordinate the major trauma pathway for complex Orthopaedic major trauma patients from admission right through to their rehabilitation and onward journey. You must be organised and methodical, work effectively within the wider multidisciplinary teams and also be able to use your own initiative and clinical judgement in providing the right care and decision making regarding complex trauma rehabilitation pathways. The complexity of the care and rehabilitation pathways for these major trauma patients makes this role very exciting and with capacity for innovation, implementing service development projects and developing the MSK rehabilitation pathway within the Major Trauma Centre. We require someone with equal knowledge and skills to drive excellent patient care forward and be a true patient advocate. You will need to perform at an advanced level to support the major trauma pathway and can demonstrate their ability at this level through: Expert clinical practice Leadership and management in order to improve and transform services Education and development of self and others This is a permanent post and secondments will also be considered. Main duties of the job Responsible for the coordination and facilitation of the major trauma rehabilitation pathway across the Major Trauma Centre, linking in with relevant professionals across the Major Trauma Centre and the Major Trauma Network. Identify the projected on-going specialist and non-specialist rehabilitation needs for major trauma patients at an early stage of admission and support the onward pathway planning and post discharge follows-up. Coordinate each element of Inpatient transitions of rehabilitation care between clinical and therapy teams and coordinating each phase from Inpatient to Outpatient's follow-ups. Ensure all major trauma patients have a completed rehab prescription in line with national standards, ensuring onward plans and goals are articulated to onward acute providers or Community teams. Ensure the process and completion is coordinated and the Rehabilitation Prescription is verified prior to the patient discharge. Implement and audit key performance indicators relating to the management of trauma patients in the rehabilitation and re-enablement end of the pathway. Establish and develop relationships. Assist with the process to benefit the patient experience and outcomes, improve patient flow through reducing length of stay. Be a source of information and advice for the Major Trauma Service and Inpatient teams, patients and relatives.
