MAIN DUTIES AND RESPONSIBILITIES
Clinical Governance
To develop and take an active role in quality monitoring of the service, including leading research projects and audit activity with the aim of improving the service/service area. Partnership with service users and carers is integral to these activities. The Post Holder will evaluate and review research and relevant literature and be aware of the implications of social and local policies, procedures, and information packages related to service delivery. Will have the ability to work under pressure and manage staff and services.
Management and Leadership
The Post Holder will act as a role model for RETHINK staff, providing support and guidance to maximize the experience of clients at Fieldview. The Post Holder will take responsibility for utilizing a significant number of resources, including four crisis beds, access to in-patient services, processing referrals, and allocating activity to achieve a rapid response to crisis referrals. Will plan and organize workloads for staff, including staff allocation, planning patient care, and group activities. The Post Holder will supervise day-to-day care in their service area, including Crisis, Police, Acute Mental Health, Home Treatment, and Specialist Care, ensuring staff are designated to appropriate areas. Additionally, the Post Holder will be involved in recruitment, selection, appraisals, and supervision of staff.
Education and Development
Acute Mental Health is a learning environment that accepts short-term placements, qualified practitioners, and various agencies wishing to gain insight into Mental Health Services. The Post Holder is expected to be involved in providing internal training sessions and presentations to other services and students. The Post Holder may be asked to provide specific training or mentorship to students. Team members are expected to become actively involved in the change process and demonstrate commitment to changing roles, including the development of specialist protocols and specialized programs of care.
Team members will provide continuous development and evaluation of patient information packages. Individuals within the team will provide teaching and education to service users and carers regarding living with mental ill health and accessing service providers. This will involve working closely with other care providers and relatives in a constructive manner using education, advice, and support. The Post Holder will organize and manage their own clinical supervision and supervise peers within Navigo agreed guidelines.
Clinical Patient Care Duties and Responsibilities
To provide high-quality care in either the home, Fieldview, or hospital setting. Clients referred to the service will usually have complex needs and problems. Families/carers will also need appropriate support, and there will be regular liaison/referral to other agencies. This will require capability in the following areas:
1. To maintain accurate records, both written and computerized, to a standard that reflects their level of clinical expertise. This may involve using specialized psychometric tools to complement the assessment and evaluation of client care.
2. The Post Holder must have experience using psychosocial interventions in clinical practice.
3. Liaison with community-based services and resources such as education, Social Security, employment, housing, Police, and voluntary sector organizations to facilitate the delivery of the most appropriate and best possible care for the individual client, carers, and their families.
4. Discuss complex issues surrounding risk management and plan care accordingly, advising others regarding the management of care.
5. Discharge planning.
6. To respond to client crises in a positive, proactive manner.
7. To develop clear care pathways, including methods to reduce the risk of further crises developing.
8. To work flexibly according to client needs.
9. To provide telephone support to any clients, carers, outside agencies, or clinicians who require information, advice, or crisis intervention. This will require the Post Holder to make immediate, effective decisions that will have implications for the outcomes of the clients' mental health.
10. Be able to assess complex mental health conditions, then develop and implement care packages in various settings.
11. If required (or at their request), undertake Mental Health Act Training (AMHPs) and, if qualified, take part in the rota.
12. To assess adolescents out of hours and refer to appropriate services.
13. To assess if required in the Police Station.
14. To be part of the on-call rota, covering 7 days a week, 24 hours a day on a rota.
Scope and Practice
The Post Holder will be expected to take responsibility for a number of tasks including:
1. Process referrals, prioritize, and assess risk management of clients who are in crisis and may display high-risk behaviors.
2. The Post Holder will be the first line contact for all mental health at all times to the general Trust and primary care (in times of crisis).
3. The Post Holder will define the level of acute mental health crisis and the use of resources, including the decision to admit a person to either the acute in-patient beds or Fieldview beds, processing general practitioner referrals, and urgent CMHT referrals.
4. The Post Holder must understand relevant policies and their application to related face-to-face physical aggression and associated actions, e.g., COSHH, Infection Control, exposure to body fluids, and exposure to both verbal and physical aggression.
5. The Post Holder will be expected to professionally contribute and/or lead and manage serious untoward incidents, challenging behaviors, ensuring all parties are debriefed and supported.
6. The Post Holder will be expected to be involved in and implement new and revised policies within their own sphere of work.
7. Will offer clinical supervision to staff from their own discipline.
