Countess of Chester Hospital NHS Foundation Trust, staff pride themselves on not only leading through clinical excellence, but also by creating an environment where all staff are valued and appreciated. To achieve this, the Trust has embarked on an exciting new strategy focused on continuous improvement.
Our vision is to improve the lives of our community and provide excellence in healthcare, through partnership and innovation
Our High Performance Culture will support everyone to be the best version of themselves by being true to our Trust vision, values and behaviours.
We welcome applications from colleagues from our local and neighbouring communities of all backgrounds and identities, who are currently under-represented within our region such as Black, Asian, Minority ethnic and those with disabilities.
As a region we are reviewing the way we work, and exploring how new working practices that support flexible working can create a positive work-life balance. As part of our recruitment process we will explore how our inclusive employment policies, flexible working, staff engagement forums, Trust facilities and services can be of mutual benefit., The National Health Service (NHS) has a number of sub-acute services that have developed in recent years from the guidance of NHS white papers such as the NHS Long term plan (2019) and NHS five year forward view (2014), which recommend for NHS trusts to deliver more care and services closer to a person's home rather than in acute hospitals.
As pressure on emergency departments across the UK grows, new care models will be pivotal in delivering and relieving some of these pressures by diverting work away from the hospital and into the community through the 2-hour Urgent Community Response and virtual ward offers.
In Cheshire West, the 'Hospital at Home' service has implemented virtual wards allowing patients to get the care they need at home safely and conveniently, rather than being in hospital. The NHS is increasingly introducing virtual wards to support people at the place they call home, including care homes.
The Hospital at Home workforce fits the NHS model well in supporting hospital level care at home; experienced General Practitioners, advanced clinical practitioners with prescribing rights, registered therapists / nurses and support workers offer a 7-day service that runs from 8am-8pm. Allowing for wrap around care and an excellent patient experience as evidenced in the services monthly feedback from patients., We are looking for a hard-working, enthusiastic, highly motivated and experienced Registered Nurses to join our Hospital at Home team to support this innovative way of working, to cover a maternity leave.
We would welcome applications from registered Nurses with experience in acute medicine as well as community experience in the management of long-term health needs of our ageing population.
The primary focus of the role is to provide patient-centred clinical care, encompassing the skills of assessment and diagnosis of patients and providing ongoing acute management under the guidance of the GP's and Advanced Clinical Practitioner's within the Hospital at Home team. The posts are intended to provide improved outcomes for our patients by providing safe, kind and effective care.
If you are looking to join an innovative service at the forefront of real system change, then this is certainly an exciting time to join our existing team.
