We are currently seeking Registered Mental Health Nurses to become part of our dedicated Community Mental Health Team. Based at Saffron House in High Wycombe, our team is committed to providing exceptional care and treatment to adults with mental health challenges. Our goal is to deliver patient-centred care of the highest quality, while supporting our staff in their roles and fostering their professional growth.
As part of the Chiltern West CMHT Adult Mental Health Team, you will work alongside a multidisciplinary team that includes medical professionals, psychological therapists, social workers, occupational therapists, RMNs, and support workers. We also collaborate with third-sector workers who specialise in employment, housing, and social needs. To bridge the gap between patients and clinicians, we have also introduced Peer Support Workers who bring the invaluable perspective of lived experience.
Your responsibilities will include:
1. Working as an autonomous practitioner within a multi-disciplinary team to perform comprehensive assessments in collaboration with service users and their families/carers.
2. Partnering with service users and their families/carers to create and execute treatment plans that ensure safety, promote well-being, and support independence and community inclusion.
3. Providing interventions following NICE guidelines to promote recovery and prevent relapse in various settings, regularly reviewing care and outcomes.
4. Supporting and supervising junior team members, acting as a mentor/preceptor to student nurses/trainees, and participating in clinical education and practice development initiatives.
5. Engaging in operational planning, policy implementation, and service development within the team, including leading on delegated projects and participating in research and audit projects.
