We are seeking a highly motivated and experienced Neonatal Nurse to coordinate neonatal discharges in our unit. This post offers the opportunity to work in a level 2 Neonatal Unit (NNU) and Transitional Care Unit (TCU) based in the postnatal ward.

Main duties of the job
? Follow the discharge pathway within the neonatal service and provide specialist advice and guidance to colleagues about the discharge process
? Facilitate seamless discharge of babies with complex needs from the neonatal unit (NNU) and the transitional care unit (TCU) by liaising with pertinent agencies outside the organisation.
? Take responsibility as the care provider for a determined group of patients without direct supervision.
? Supervise and teach junior staff and pre and post registration student whilst maintaining a safe working environment.
? Work within the codes of practice, professional guidelines, and policies.
? Promote the continuous improvement of clinical standards in conjunction with the application of evidence-based practice
? Utilise a variety of strategies to communicate effectively with the baby's family/carers, the multidisciplinary team, other departments within the Trust and relevant agencies outside the organisation Neonatal Matron
? Be responsible for the information governance regarding discharges for NNU/TCUand the completeness of data
? To be highly competent, knowledgeable practitioner ensuring provision of excellent evidence-based care.

1. RN /RCN with current registration
2. Degree or diploma in adult or children's nursing
3. Breastfeeding/Lactation course/training
