Within Swansea Bay UHB, we recognise that frailty is associated with increased ill health, mortality, healthcare use, care dependency and reduced quality of life and have identified a high proportion (40%) of our acute medical admissions are frail.
People with frailty often experience long waits in overcrowded admission areas. Spending extended periods of time in these types of environments is challenging to their senses and increases immobility, which leads to deconditioning and delirium. In an attempt to move people quickly out of admission areas like ED (emergency department), we are not always able to offer older people a frailty assessment before they are admitted. By failing to complete a frailty assessment (ideally within 30 minutes of their arrival at hospital), patients are at greater risk of hospital-acquired harms.
Main duties of the job
Morriston has an exciting opportunity for an Advanced Clinical Practitioner to work within the frailty unit, which is our new front door service. This role involves providing care to patients presenting with frailty syndromes, injuries, illness, and acute exacerbation of chronic conditions.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Job responsibilities
The Advanced Clinical Practitioner will be assessing, diagnosing, and managing their patients autonomously and as part of the MDT. We are looking for a dynamic Advanced Clinical Practitioner with an interest in frailty to work within the frailty unit. We aim to provide a dedicated Frailty Unit staffed by a multi-disciplinary team of geriatricians, nurses, AHPs, and pharmacists who will provide the targeted care these vulnerable patients need, right from the offset.

* Registered Practitioner (e.g. NMC/HPC)
* Relevant Masters Degree
* Accredited as an Advanced Practitioner
* Willingness to undertake further Masters level education relevant to the role.
