Your role
Reporting to the Head of Nursing, this role will be responsible for the safe and effective planning of nurse visits, acting as 1 st line escalation for clinical decision making regarding the delivery of patient care.
Working closely with the planning coordination and nursing team, upon escalation, you will act in the capacity of secondary assessment to determine if treatment can proceed or whether further guidance is sought from the patient's clinical referring team.
Where patients contact Pharmaxo Healthcare for clinical queries, you will liaise with the patient and where appropriate signpost to their relevant clinical team or liaise directly with their referring centre, ensuring at all times that you operate in accordance with our contractual obligations and your clinical competency.
Also w orking with the wider nursing and quality teams you will seek to increase efficiency in processes whilst maintaining safe, effective nursing care.
