To manage the care of patients with long term conditions by assessing, planning, implementing, and evaluating treatment plans for patients with long term conditions (or at risk of developing one) that meet identified needs and promote health and well-being. Provide the full range of nursing services as required to meet the needs if the practice, including venepuncture, ECG measurement, blood pressure measurement, urinalysis, immunisations (children and adults including travel), injections, cervical smears, swabs, travel services, assessment of risk factors, lifestyle advice, weight management, ear care, suture removal, wound care, doppler assessments, family planning, etc. Educate and support patients as needed, including adopting healthy lifestyles, appropriate self-care To assess the needs of patients, providing appropriate care/treatment in conjunction with other health care professionals, or independently, as appropriate. Refer to colleagues or other agencies as appropriate. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. Support initiatives, including contacting patients, to improve uptake of screening, vaccinations or other services and support the achievement of targets (e.g. under the Quality and Outcomes Framework). To maintain accurate and contemporaneous records to relevant professional standards/guidelines. To ensure awareness of statutory and local child and adult protection procedures, including systems of referral. Ability to recognise signs and symptoms of child abuse. Administration and professional responsibilities Participate in the administrative and professional responsibilities of the practice team Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes Ensure the clinical computer system is kept up to date, with accurate details recorded and amended Ensure appropriate items of service claims are made accurately, reporting any problems to the practice administrator Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit Attend and participate in practice meetings as required Restocking and maintenance of clinical areas and consulting rooms To assist the doctor to carry out minor operations as required. Report any problems to the Practice Manager Supplies and equipment Maintenance of equipment and stock relating to patient care Training and personal development Training requirements will be monitored by yearly appraisal and will be in accordance with practice requirements. Personal development will be encouraged and supported by the practice. It is the individuals responsibility to remain up to date with recent developments. Participate in the education and training of students of all disciplines and the introduction of all members of the practice staff where appropriate Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development, ensuring PREP requirements are met. If it is necessary to expand the role to include additional responsibilities, full training will be given.
