6 Month Fixed-Term Contract
We have an exciting opportunity for a Physiotherapist to join our Intermediate Care Service based at Ely on a six months Fixed Term Contract.
The community is the place to be! Rehabilitation in the community is at the forefront of the implementation of Discharge To Assess. You may have gained experience in the acute hospitals and now your career in the community beckons. This role is based in the therapy led Intermediate Care Service where you will work with the wider multidisciplinary team to rehabilitate patients, discharged from hospital and to prevent admission, in their own homes and in care homes. You will be based in Ely but work closely with your colleagues in other localities and across the system that provide Intermediate Care for the population of Cambridgeshire and Peterborough. We offer excellent professional support, and you will have the opportunity to rotate to other specialist teams within CPFT such as in-patient rehabilitation, neuro and neighbourhood team community therapy.

As part of the role, you will:
• provide assessment and physiotherapy interventions to patients in their own homes and care homes.
• implement therapy plans as well as delegating to therapy assistants and integrated care workers.
• develop your skills in sub-acute rehabilitation and discharge planning in the community working with colleagues in health, social care and the voluntary sector.
• work autonomously in the community setting travelling across the community seeing people in their own homes
