Ward Tracker - Haematology/Oncology Inpatient Ward

Job overview
The Haematology/Oncology inpatient ward is a 20 bedded ward specialising in acute care of patients with haematological and oncological conditions. The role will involve supporting the clinical staff in the smooth running of the ward, including general ward clerk duties. The post holder is required to work collaboratively with the multidisciplinary team to ensure safe and timely patient discharges.
Main duties of the job
1. Support the ward manager and ward co-ordinator/NIC in maintaining patient flow.
2. Support clinical staff in keeping e-track up to date.
3. Liaise with the duty manager when a bed is available.
4. Attend Whiteboard rounds, identify actions required and actively manage agreed discharge plans for patients.
5. Maintain whiteboards and e-track information to ensure it is up to date.
6. Arrange or photocopy notes for patient transfer.
7. Assist in the completion of discharge checklists for accurate discharge planning.
8. Chase doctors for completion of discharge letters and deliver TTOs if necessary.
9. Ensure diagnostic requests are expedited and escalate delays.
10. Arrange appropriate transport for discharge.
11. Ensure patients can complete a friends and family feedback survey.
12. Utilise IT systems to secure and record timely and accurate data relevant to the role.
13. Coordinate admissions and transfers in the wards and to other organisations.
14. Complete the filing of patient notes when discharged.

Person specification
Education
* Good standard of literacy and numeracy
* NVQ Level 2 or equivalent experience
Experience
* Previous Customer Service experience (minimum of 12 months)
* 1-year previous administration experience to NVQ level 2 or equivalent
* 1 year experience of working in an ED/healthcare environment
Knowledge
* Clear, concise verbal and written communication
