Job Introduction
To provide effective nursing care for patients in their own homes and other community settings. The role involves clinical care and treatment for patients requiring wound care management, medication administration, and catheter care, as well as providing support for patients with life-limiting illnesses and their families, ensuring they receive palliative care to remain at home during their final weeks of life. Collaboration with the 24/7 community nursing teams and developing strong working relationships with GPs, multidisciplinary teams, and social care services is essential to deliver high-quality care that maintains the dignity and respect of individuals while promoting their comfort and well-being.
Main Responsibility
1. Deliver care within operational and clinical protocols that support safe, high-quality nursing practice, including assessment, developing care plans, and maintaining associated records.
2. Contribute to the leadership, management, and organization of the community nursing team, ensuring evidence-based and clinically effective care.
3. Deliver high-quality nursing care appropriate to the grade and experience, working alongside the clinical teams across community services.
4. Accept delegated nursing tasks from the senior nurse or paramedic coordinator on duty and escalate concerns appropriately to senior colleagues.
5. Promote a person-centered approach, ensuring service user involvement, autonomy, and choice in clinical care, providing advice and guidance when necessary.
6. Complete written records as required by the service, NMC standards, and partner agencies.
7. Ensure all service users have an up-to-date evidence-based care plan that meets their needs.
8. Maintain and value a multidisciplinary approach.
9. Explore opportunities to enhance health promotion interventions.
10. Utilize evidence-based practice as the foundation for assessment, planning, implementation, and evaluation of nursing care.
11. Challenge practices that require change or development or do not meet required standards.
12. Deliver personalized care, ensuring dignity and respect are maintained at all times.
13. Strive to improve clinical practice based on research, professional development, and reflection.
14. Participate in teaching and assessing pre and post-registration students.
15. Deliver care within Great Western Hospitals NHS Foundation Trust Equity and Diversity protocols.
16. Address concerns and complaints promptly, liaising with PALS and complaints personnel.
17. Engage in the implementation and progressive use of information technology within the service, ensuring compliance with the Data Protection Act.
18. Adhere to local safeguarding protocols and procedures to protect vulnerable adults and children from significant harm.
19. Contribute to the development of nursing practice using informed research and further education as appropriate.
20. Have suitable transport available which is fit for purpose.
21. This job involves substantial access to vulnerable adults and/or children, and an Enhanced Disclosure & Barring Service check (DBS) is required.
The Ideal Candidate
1. RN (Adult) first or second level registration and currently registered with the NMC.
2. Systematic approaches to assessment, planning, and evaluation of needs-led care.
3. Able to carry out clinical activities of a fully functional nurse, with experience in wound care, catheter care, palliative care needs, diabetes, and injections.
4. Good written and oral communication skills.
5. Knowledge of legislation affecting professional practice.
6. Good IT skills.
7. Ability to present application and supporting information clearly and reflectively.
8. Flexibility and sensitivity in work.
9. Ability to work well in a team and independently.
10. Excellent organizational skills.
11. Adaptability to a combination of roles, including visiting patients at home and using a patient record database to review physiological signs and symptoms.
12. Willingness to work in other areas of the Trust as required.
13. Ability to drive and have access to a vehicle or equivalent mobility.
14. Availability to meet service delivery needs seven days a week.
