The Newcastle upon Tyne Hospitals NHS Foundation Trust
As part of Newcastle upon Tyne Hospitals strategy to ensure safe and effective patient flow, we are planning to open a discharge lounge at Freeman Hospital and to enhance our current discharge lounge at the Royal Victoria Infirmary. We are therefore looking to recruit two enthusiastic, motivated Band 6 Registered Nurses to take lead roles in ensuring the success of this initiative.
This is an exciting opportunity to play a key role in supporting patient flow and increasing inpatient bed capacity, whilst ensuring the provision of high quality care and improved patient experience for patients being discharged from Newcastle upon Tyne Hospitals.
The successful candidates will be required to liaise with patients, families, carers, ward staff, internal departments, and external agencies to ensure that patients being discharged via the discharge lounge experience a coordinated journey from ward to discharge lounge to home.
The successful candidates must be strong team players who have the ability to develop close working relationships with key stakeholders. These roles require an ability to work flexibly on a daily basis to meet needs and pressures of the service, with the ultimate goal of ensuring the provision of seamless patient flow.
We are recruiting to two posts, one at Freeman Hospital and one at Royal Victoria Infirmary; however, there will be a requirement to provide support across sites as required by the service.
Main duties of the job
The key duties of the role are to identify patients who are suitable for discharge via the discharge lounge and actively support the ward teams in ensuring a safe discharge pathway for all patients. The post holder will be required to work closely with patient services coordinators to prioritise patients to transfer to discharge lounge, ensuring in doing so that patient flow is maximised. You will be required to actively promote and encourage the use of the discharge lounge across both sites.
