Job summary
The Care Coordinators will work as partof a growing multi-disciplinary team within general practice, supportingpatients from a number of practices in their Network. This role will give youwill have the chance to shape the service and the role. You will be workingwith patients to help them understand their conditions and what matters tothem, and match the support that is available locally to keep them happy andindependent. This might involve working with clinicians looking after localCare Homes, organising PCN multi-disciplinary team meetings for clinical andnon-clinical staff or working with a specific cohort of patients yourself perhaps frail elderly or a particular long term condition.

The role will require high standards andgreat attention to detail, in order to provide an excellent service to patientsand colleagues. Main duties of the job
supportthe MDTs within the Primary Care Network (PCN) to deliver effective,co-ordinated care for specific patient groups
supportthe PCN to deliver effective, coordinated care to the care homes in theirlocality, working with the Clinical lead and other partners
liaisewith other clinical and no-clinical professionals within the team and otherpartners involved in patient care locally
utilisepopulation health intelligence to proactively identify and work with a cohortof patients to deliver personalised care
holisticallybring together all of a persons identified care and support needs, and exploreoptions to meet these
helppeople to manage their needs through answering queries, making and managing appointments,and ensuring that people have good quality written or verbal information tohelp them make choices about their care
providecoordination and navigation for people and their carers across health and careservices, working closely with social prescribing link workers, health andwellbeing coaches, and other professionals
supportthe coordination and delivery of MDTs within the PCN
promotethe role and MDT within the Network
Toensure that action points identified within the MDT are recorded and followedup
