Responsibilities
Conduct comprehensive assessments of client needs and strengths.
Develop and implement personalized support plans in collaboration with clients and their families.
Provide direct counseling and crisis intervention services to clients experiencing social or emotional difficulties.
Collaborate with healthcare providers, community resources, and other stakeholders to ensure holistic support.
Maintain accurate and confidential client records in accordance with legal and ethical standards.
Advocate for clients' rights and access to necessary services and resources.
Monitor client progress and adjust support plans as necessary to promote successful outcomes.
Requirements
Master's degree in Social Work (MSW) from an accredited institution.
Current licensure as a Social Worker in the relevant state or region.
Minimum of two years of experience in adult social work or related fields.
Strong understanding of social policy, community resources, and support services for adults.
Excellent communication, interpersonal, and organizational skills.
Demonstrated ability to work collaboratively within a multidisciplinary team.
Commitment to ongoing professional development and adherence to ethical standards in practice.
Hours Per Week: 37.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £19.85
