We are looking for an enthusiastic and motivated Band 6 Mental Health Practitioner to join our dynamic Community Mental Health Team. In this role, you'll provide high quality, compassionate care to older adults with both dementia and functional mental health conditions. You'll work collaboratively with a well established team to deliver personalised treatment plans, conduct comprehensive assessments and provide support to patients and carers as they navigate complex mental health challenges. The role may also involve occasional working with working age adults to support the wider goals of the service.

To work with clients experiencing dementia or functional mental health concerns, and their families, in the Older Adults’ Community Mental Health Team within East Cheshire. The post holder will hold a professional qualification and have relevant experience regarding older adult mental health care.
The post holder will be responsible for:
o Undertaking assessments and coordinating the care of service users.
o To work with family members closely to maintain high standards of collaborative care.
o To actively contribute to the day to day operational functions of the team such as completing new assessments, covering office “duty” and working as a team to deliver the team’s goals.
o To develop and maintain positive working relationships with service users, their carers, General Practitioners, third sector partners, other significant professionals, and members of the multi-disciplinary team.
o Ensuring that clear channels of communication are established and maintained with clients, carers, GPs, Social Services and other partners.
o To promote and identify areas requiring development within the CMHT in the Cheshire East area and to take a lead role in the development of services in conjunction with the Team Operations Manager, Head of Clinical Services and other professionals.
CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care – including the provision of three GP surgeries.
