Band 6 - Deputy Team Lead - Specialised District Nurse
Do you want to join an outstanding team of community nurses and therapists in Newham? We are looking for two B6 District Nurse's to join our team who is passionate about community nursing. You will be supported by a wide number of healthcare professionals, including DN and therapy clinical leads, a deputy lead nurse and lead nurse, pressure ulcer improvement facilitators, palliative nurses, a referral and assessment team, a rapid response team, care navigators and telehealth practitioners.
You will receive monthly supervision with your Team Leader and participate fully in a monthly senior nurses meeting. You will play a vital role in ensuring the service operates at the highest level, e.g. by attending the monthly management, pressure ulcer and safeguarding meetings. You will also be able to put into practice and develop your clinical and managerial supervision skills and engage in QI projects.
Main duties of the job
To have responsibility for the nursing care given to the patients on her/his own case load and for the nursing care provided by the team. The District Nurse will ensure a case management model of delivery including the effective assessment, planning, implementation, evaluation of care and the discharge of patients from the case load meets the professional and clinical standards/guidelines at all times under the direction of the District Nurse Team Leader.
The District Nurse will ensure that a high quality service is provided in patient’s homes, clinics or residential care settings within available resources. The service is provided over 2 shifts working from 8am till 8 pm.
He/she will be responsible within his/her team for the liaison and effective and cooperative working arrangements with General Practitioners and other agencies.
