An exciting opportunity has arisen to lead our established Older Persons’ Specialist Team (OPST) within Division B – Medicine for Older People.
The successful applicant will be joining a team of highly motivated, experienced, specialist nurses. The post will be based with OPST but working alongside the medical teams and ward & therapy teams to support patients over 80 who have complex health and social care needs.
This will be a permanent position working 22.5 hours per week.

WHAT YOU’LL DO
Within the scope of the role, the Lead older persons practitioner will assist the medical and nursing teams in managing caseloads of patients who have complex health and social care needs and work closely with the community teams in the management of long-term conditions.
This is an innovative opportunity for integrated working across both primary and secondary care providing a high quality, evidence based seamless service for patients.
WHAT WE’RE LOOKING FOR
We are looking for an enthusiastic, motivated leader to join a team of dedicated staff within our medicine for older people care group.
You will provide expert clinical knowledge and support to both the medical and ward/department teams with the management of these groups of patients.
We pride ourselves in the provision of high standards of patient care and support for our nursing and medical teams.
You should be energetic and forward thinking, as well as eager to develop your own clinical and management skills.
Proven evidence of excellent inter-personal skills and the ability to work across boundaries is essential.
