Job Role: Qualified Social Worker Adults – Job Location: Bath and Northeast Somerset Council Salary: £38/hour Working period: Monday – Friday Hybrid Working Working Hours: 37 hours per week Assignment period: 3-6 Months Ongoing Complex Intervention and Treatment Team Conduct comprehensive assessments of social, physical, and mental health needs and develop person-centered care plans in collaboration with service users and professionals. Work with healthcare providers and participate in case meetings, ensuring integrated, holistic care through communication with other service providers. Provide direct interventions, such as emotional support, crisis intervention, and daily living assistance, while supporting the management of medical and mental health conditions. Respond to emergencies, develop crisis prevention strategies, and ensure access to emergency services as needed. Regularly review and adjust care plans based on client progress and needs, maintaining accurate case notes and tracking outcomes. Act as an advocate for clients’ rights, support access to resources, and promote client independence through education and empowerment. Provide training on complex care needs and best practices, while staying updated on relevant policies and practices. Identify and act on safeguarding concerns, working with safeguarding teams to ensure client safety. Complete detailed documentation in line with policies and prepare reports for internal and external reviews. Coordinate resources, including services, equipment, and funding, and manage care package budgets while ensuring access to appropriate support services.
