This is an exciting opportunity to join the Gloucestershire Maternity workforce as a Birth Review and Reflect Co-ordinator. We are looking for an enthusiastic, motivated, and experienced individual to help launch a new aspect of our service.
The post holder will work closely with the Consultant Midwife, Clinical Governance Team, and Patient Experience Team to support the set up and co-ordination of a Birth Review and Reflect service for people who have given birth within Gloucestershire Hospitals NHS Foundation Trust.
Main duties of the job
1. Act as a single point of entry for women to access maternity services for a birth discussion following the birth of their baby.
2. Make initial contact with the woman within agreed timescales and after having an initial conversation will ensure the woman is seen by the most appropriate professional.
3. Keep in contact with the woman, advising her of appointments, waiting times and onward referral to any other services, e.g., Birth Anxiety and Trauma Service, PALS and complaint teams.
4. Provide efficient, confidential and professional administrative support to professionals who are providing Birth Review and Reflect consultations.
5. Collate key themes and issues relating to patient care and experience and establish mechanisms to report these back to the Maternity Experiences workstream, Clinical Governance Team and also escalate any concerns appropriately.
6. Obtain feedback from service users who have used the Birth Reflect and Review service, collating feedback to shape service improvement.
7. Work with Communication Teams and Maternity and Neonatal Voices Partnership to ensure service users are made aware of the service.
8. Promote the service within the system, explaining the role of the service to professionals including GP's and Health Visitors.

Job responsibilities
1. To contribute to the set up and effective running of a Birth Review and Reflect Service.
2. Ensure database and systems are in place to monitor themes and outcomes for women accessing the service and these are reported in a timely manner to the Clinical Governance team.
3. The post holder will ensure the smooth running of the service with responsibility for receiving all referrals, responding to referrals within stated timescales.
4. Offering a prompt initial contact and support to the woman, and remaining an accessible point of contact, and support throughout involvement with the service.
5. Co-ordinating appointments with professionals and keeping families informed following self-referral into service or referral by other health care professionals.
6. To support professionals prior to Birth Reflect and Review consultations with women and birthing people, ensuring that relevant maternity notes and records are available.
7. Organising appointments in confidential space individual women feel comfortable to attend including hospitals, their own homes and community hubs.
8. To support professionals and service users following appointments this may include written and verbal feedback and supporting referral process onto other services such as PALS or patient complaint teams.
9. To contribute to the service user experience ensuring women and families are listened to and responded to with compassion.
10. The post holder will support the feedback loop ensuring that the service is able to identify themes and learning from the experience of service users which will be used by the Maternity Service and wider LMNS to improve the experience for service users.
11. To contribute to the safety culture and service improvement within the organisation, ensuring that feedback is used to improve quality and safety of maternity services and reduce inequalities in experience and outcome for women.
12. To liaise with a variety of professionals and organisations to ensure women referred to users to the service are responded to in a timely manner and supported through any onward referral pathways.
13. Contributes to the safeguarding of mothers and babies being aware of triggers for concerns and reporting concerns appropriately within the Trust and multi-disciplinary team.
14. Maintain accurate patient records in line with Trust guidelines. Ensure that any database or client related data are correctly stored, processed and secured in line with departmental and Trust policies and to comply with IG requirements. Referral attendance and outcome records are kept and analysed as part of service evaluation and development.
15. To contribute to the monitoring and evaluation of the service, including collating themes, outcomes and feedback from service users.
16. Liaise closely with Perinatal Mental Health Services, Health Visiting and GP services to ensure smooth transition from the service and handover of care.
17. To organise own time, and to work flexibility to best support service delivery, meeting set targets and deadlines as defined by the service.

Qualifications
Essential
* Educated to NVQ level 2 or above standard of numeracy and literacy.
* Degree level of education or equivalent experience.
* Quality Improvement training or willing to undertake training in post.

* Post Graduate Management Qualification.
