The virtual ward has a dedicated team supporting the Frailty team in both the Emergency Department and across all the acute medical wards. The Frailty team is a multi-disciplinary team based in the Emergency Department and the trust is currently looking to create and run an Acute Frailty Assessment unit delivering same day emergency care for patients with frailty as part of the frailty approach across the hospital. The successful candidate will support the implementation of the Frailty SDEC alongside the clinical and operational teams. This is a new initiative for East Cheshire Hospital to support the aging population of Eastern Cheshire. Hours per week: 10 PA's Main duties of the job Provide a high quality, safe and compassionate care to the acutely ill medical patients admitted to our hospital and provide them with the best possible experience during their admission. Active participation in Trust Clinical Governance activities, including audit, mortality review and appraisal process.
