Job Title: Adult Social Worker - Hospital Discharge Team
Location: Shropshire
Rate: £32.20 per hour
An exciting job role has recently become available for an Adult Social Worker to join a dynamic Hospital Discharge Team based in Shropshire.
What will your responsibilities be?
In this role, you will provide the best service for individuals who require support with complex discharge and admission avoidance from the Emergency Department and Frailty Assessment Unit under the principles of the Care Act (2014). With the help of the model, you will aim to get people home as soon as possible to improve length of stay, enhance flow in the hospital, and as a system, support the hospital to have the best performance and quality for the people who need acute intervention.

To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
