Provide holistic and evidence based advanced nursing care to children and young people with complex palliative care, pain and symptom control needs using a care pathway approach delivered to the child and family at their preferred place of care. Manage a designated caseload within a geographical locality As part of the Team act as a point of contact for specialist symptom control and management advice for all children and young people throughout the palliative care pathway within Hertfordshire, Bedfordshire or Milton Keynes. To participate in an On Call Rota system, which ensures; - the team are responsive to families at all times, when a child is on the Red caseload. - specialist telephone advice and support is available to any family or professional involved in the care of a child with a life limiting condition - advice and support for staff on the childrens in patient unit - In exceptional circumstances the on call nurse may be required to go into the hospice to support a childs care or in the event of staff sickness where no other staffing can be sourced Act as a non medical prescriber within the scope of your professional practice Support existing services to provide a co-ordinated multidisciplinary approach to the provision of palliative care for children Work collaboratively with an identified group of medical practitioners to provide symptom management advice and support Contribute to Keechs involvement within the East of England Managed Clinical Network Ensure that all children who need them have current symptom management plans in place and that these are regularly reviewed Support integration of Advance Care Planning for children and young people in line with East of England Policy and Guidance Provide support for care coordination/key working/lead professional role responsibilities Advocate for and support parents, children and young people Act as a positive role model which enables change. Advise and support patients and their families in a range of settings, maintaining an holistic approach, in accordance with Keech hospice philosophy Demonstrate a research based approach to nursing and participate in on-going research and audit Identify risk and lead risk management involving patient care and lone working and implement learning outcomes from recorded incidents. Work confidently and competently as a lone worker and within a team in the home setting and within the hospice when required Network at a local and regional level Provide clinical leadership for the childrens community service and line management nurses Use current evidence of best practice to develop care provision Contribute to the development of evidence based standards, guidelines, protocols, policies and audit ensuring adherence to recommendations of agreed documentation Support caseload management across the team, assisting with admission and discharge processes Take responsibility for overseeing mentorship of new staff and ensuring baseline competencies are achieved and signed off in a timely manner, e.g. syringe driver competency. Receive and disseminate information related to paediatric palliative care through attendance at team and hospice meetings, and through liaison with other relevant professionals. Assist in maintaining patient statistics by keeping up to date patient notes and administrative records. Participate in the development, implementation and evaluation of Keech hospices care strategy as required. Lead on service development or project management as identified in conjunction with the Associate Director Patient Services (Children) and Clinical Director Deliver learning and development to other health professionals to enhance knowledge, skills and confidence and increase capability across services. EDUCATIONAL Participate in Clinical Supervision for own development and support. Provide Clinical Supervision for nurses and HCAs. Participate fully in the orientation/mentorship of new staff, volunteers and students Regularly read and update personal knowledge of Paediatric Palliative Care Nursing. Liaise with Clinical Development Lead and Associate Director Patient Services (Children) to ensure an on-going programme of paediatric palliative care education is available Deliver learning and development to help enhance knowledge, skills and confidence and increase capability in generic and core services to reduce the need for specialist intervention
