We are looking for anenthusiastic band 5 community staff nurse, to work with the OOH Districtnursing service 3 evenings per week, working to
You will join a team of nursesdelivering high quality care to patients who are housebound.
Youwill be caring for patients with long term conditions and complex health needs,developing your skills and knowledge, whilst working towards an integratedhealthcare structure. You should be able to demonstrate you can make clinicaldecisions to provide evidence-based practice and care planning in response toclinical assessments.
Main duties of the job
The successful candidate will perform holisticassessments of patient nursing needs. Plan, assess, implement and evaluate caredelivery according to changing health needs utilising prescribing skills.
Ensure practice is evidence based throughundertaking audit and critically appraising literature to achieve a highstandard of care.
The post holder actively supports the DistrictNursing team by undertaking responsibility for a delegated caseload.
Liaise with the District Nurse team leader toensure information sharing in relation to patient care / changes to care planand the continuity of care and reviewing of complex care packages.
Mentoring, Teaching and Assessing.
Liaises with all health care professionals andother agencies to ensure effective service delivery.
Involve patients and carers in the planning anddelivery of care.
Maintain accurate records in accordance with NMCStandards.
To participate in the health needs assessment ofthe practice population to identify areas of unmet need.
Ability to work without direct supervision and tosolve problems.
Work on a rota basis to facilitate weekend workingacross the district nursing teams

To promote the health and well-being within thefamily unit and community as a whole.
Perform comprehensive assessment of patient nursingneeds as the named nurse.
Plan, assessand evaluate programmes of care according to changing health needs under thedirection of the District Nurse.
To support the team leader in the co-ordination of complex andcontinuing care packages.
Maintain accurate records in accordance with NMC.
Attend a multi-disciplinary case conference as the named nurse ifappropriate.
To communicate and collaborate with othermembers of the PHCT, voluntary and statutory organisations to supportindividual patient care.
To establish and maintain effectivecommunication with patients, relatives and carers in complex and potentiallychallenging situations.
To competently perform clinical skillsrelated to the post.
To be professionally responsible for adelegated caseload under the supervision of the District Nurse.
To assess health needs at individual,practice and community level.
To undertake chronic disease managementreviews using appropriate template.
To work without direct supervision.
