Job summary
The position holder will be responsible for caring for children with complex health care needs in the community setting either at home or school. This is an extremely rewarding area of practice caring for children with life limiting/ life threatening conditions. The post holder will provide hands on nursing care to these children following training and assessment of competence in various nursing skills. Full or Part time hours available.
Main duties of the job
The post holder will be required to be caring, compassionate, dedicated and flexible in approach. An ability to work well on own initiative and within a small team. The post holder will be required to work shifts including unsociable hours, weekends and nights.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

1. Work flexibly as part of a Multidisciplinary Team2. Undertake direct patient care as delegated by the professional team and within the individual childs plan of care as appropriate to competency To undertake duties appropriate to competency levels suctioning, gastrostomy feeding, tissue viability care, personal care and other designated nursing Encourage the childs participation with personal and daily domestic activities,5. 1. QCF Level 3 or equivalent qualifications/experience
2. NVQ level 3 in Health & Social care related subjects or equivalent qualifications/experience
3. Good knowledge of anatomy & physiology

4. All Wales HCSW induction framework

5. Experience of working as a HCSW without direct supervision in a clinical setting and being managed by a qualified practitioner.
6. Good understanding of risk assessment and management
