Advanced Specialist Pharmacist Acute Medicines Admissions Avoidance
With the launch of our acute medicine Virtual Ward (VW), we have an exciting opportunity to join our highly respected team of integrated advanced specialist pharmacists and technicians, providing a dedicated 7-day clinical service across our Acute Medicine footprint.
We are looking for a highly motivated, innovative, and enthusiastic individual that enjoys the challenge of working in a busy multidisciplinary environment. You will be joining a dynamic team to lead, deliver, develop, manage, and evaluate clinical pharmacy services and ensure a high level of clinical pharmacy input to the care of patients in Acute Medicine. In post, you will be an integral part of the multidisciplinary team; promoting medicines optimisation to a complex cohort of patients and play a vital role in the training of pharmacy, medical, and nursing staff on medicine.
The post holder will have significant clinical experience and strong interpersonal skills, ideally with an independent prescribing qualification or willing to work towards. Main Duties of the Job
1. Provide an expert level of clinical pharmacy services to Acute Medicine Services at The Dudley Group NHS Foundation Trust (DGH) as part of a dedicated 7-day ward-based service including virtual wards.
2. Provide interface and discharge support to reduce hospital admissions and readmissions related to medication-related problems.
3. Be responsible for the delivery, development, review, and co-ordination of clinical pharmacy services across Acute Medicine in line with standards defined by national priorities, divisional objectives, and the DGH pharmacy strategy.
4. Be responsible for all aspects of medicines management in the designated clinical areas and aid in the delivery of Acute Medicine service's key performance indicators.
5. Actively participate in multidisciplinary patient reviews, delivering and promoting high-quality, cost-effective, evidence-based pharmaceutical care. Using Person-Centred care to support patients to increase their understanding of their medicines to enable greater self-management and independence.
6. Collaborate with the other Advanced Specialist Pharmacists in leading specialist clinical pharmacy practice, sharing responsibility for project and performance; education & training; medicines optimisation activities; patient safety & governance.
7. Development, implementation, and audit of local guidelines and promote safe medicine use.

Person Specification
Qualifications & Education
* MPharm or equivalent academic qualification.
* Registration with General Pharmaceutical Council (GPhC).
* Postgraduate Diploma in clinical pharmacy or equivalent experience/qualification.
* Independent prescriber and registered with the General Pharmaceutical Council (GPhC) or willing to work towards after appointment.
Experience
* Experience of delivering clinical pharmacy service and advice relating to the safe use of medicines.
Knowledge and Training
* Significant experience post-registration and postgraduate experience in hospital Pharmacy.
Communication & Relationship Skills
* Excellent interpersonal and presentation skills, both verbal and written, to communicate highly complex drug or medicine-related information.
* Ability to work in a multidisciplinary team.
Analytical & Judgement Skills
* IT literacy including the ability to use a range of Microsoft 365 packages, PowerPoint, email, and use of the internet to research evidence-based medicine and to utilise medical databases.
Planning & Organisational Skills
* Evidence of planning, implementing, and monitoring outcomes in research, quality improvement, or audit.
