To work within the acute therapy service, developing effective clinical practice in the team in line with the current evidence base. Interventions will be within the specialty of medicine and elderly care to aid safe discharge. You will manage autonomously a clinical workload of patients/clients at a Specialist level, assessing and treating patients while maintaining clinical records.
Responsibilities include:
1. Specialist assessment and treatment skills in areas including but not limited to:
1. Physical rehabilitation through exercise and function
2. Assessment of risk for discharge
3. Complex psychosocial assessment of patients and carers
4. Discharge and transfer of care planning and coordination
5. Assessment of the home/discharge environment
6. Assessment for assistive equipment and adaptation
7. Falls assessment and treatment
8. Crisis intervention
9. Cognitive and perceptual deficits
2. Applying evidence-based knowledge within the specialist clinical area acquired through experience and academic study to a specialist postgraduate level.
3. Acting as a resource within the acute therapy service and providing advice for colleagues within the MDT about the assessment, treatment, management, and care of patients/clients with multi-pathological illness and/or complex psychosocial needs.
4. Working collaboratively with Occupational Therapists, Physiotherapists, and other professionals within the health and social care community and acute colleagues.
5. Ensuring effective care pathways and transfer of care/discharges with other members of the MDT.
6. Facilitating a seamless transfer of care and continuing therapeutic management of medical and elderly care patients between care settings.
7. Engaging in interdisciplinary collaboration within the MDT, addressing blurred boundary working where appropriate.
8. Developing specialist programmes of assessment, treatment, and intervention for medical and elderly care patients in the acute hospital setting.
9. Managing and prioritizing your own clinical workload efficiently while overseeing the levels of other team members to ensure effective service delivery.
10. Participating in the supervision process with staff and students as required.
11. Ensuring a high standard of patient care, including implementing and reviewing clinical standards relevant to the work undertaken.
12. Providing specialist assessment and advice for the multidisciplinary team and actively participating in MDT meetings as appropriate.
13. Being a manual handling key trainer.
