Assessing patients with a wide variety of mental and physical health comorbidities using a holistic approach, considering all aspects including social, emotional, and spiritual wellbeing within the hospital environment. Documenting assessments and interventions electronically in a timely and accurate manner. Working flexibly and co-operatively as part of a team.Using initiative alongside clinical skills and experience to make decisions as appropriate within a supportive and experienced team. Providing a positive image of people with complex physical and/or mental health needs; promoting independence and autonomy wherever possible for people living with dementia. Compassionate in meeting the needs of vulnerable people and their families and carers; advocating for patients and carers as required.Using clinical reasoning/problem solving skills when liaising with other professionals/agencies in order to best meet the needs of patients. Undertaking formal and informal education to increase skills and confidence around behaviour management and positive approaches to care for people living with dementia. Supporting staff to find the least restrictive approaches and interventions for patients experiencing delirium. Promoting the use of "This is Me" document to enable and empower staff to provide person centred care. Understanding risk management and underpinning legislation, especially around Mental Capacity Act.
