We are looking for an enthusiastic patient-focussed Clinical Pharmacist to join our team and be part of the clinical pharmacy service provision to designated wards and teams in the Newcastle & Gateshead area.
Progression opportunities will be available for staff once adequate experience of working into a clinical area is achieved and enrolment onto the Newly Qualified Pharmacist Pathway or an alternative post-registration qualification.
We provide pharmacy and medicines optimisation services to all our mental health, learning disability and neuro-rehabilitation inpatient areas, as well as to community mental health teams, crisis teams, psychiatric liaison and a variety of other specialist areas. Pharmacy staff are fully integrated into and regarded as valued members of the multi-disciplinary team.
Main Duties of the Job
Delivering clinical pharmacy services to designated clinical areas in line with professional, departmental and trust standards and policies including:
1. Medicines reconciliation and review on admission and discharge
2. Providing support to the team on medicines optimisation, transfer of care and shared care
3. Detailed medication reviews to provide advice on optimising the safety and effectiveness of pharmacological treatment regimens
4. Contribution to the formulation of individual care plans
5. Reviewing and updating care records
6. Therapeutic drug monitoring for specific medicines
7. Supporting patients and carers with the management of their medicines
8. Optimising medicines, taking a holistic view of both mental and physical healthcare needs
9. Physical health and wellbeing advice
10. Discharge planning and liaison with primary care
11. Participation in multidisciplinary team meetings
12. To act as a role model for other staff and to demonstrate personal conduct in a manner in keeping with a registered professional

Person Specification
Qualifications and Registration
* MPharm or equivalent first degree in Pharmacy
* GPhC registered pharmacist (or for trainee pharmacists with upcoming registration exam)
* Registered with GPhC
Knowledge and Experience
* Experience of hospital pharmacy.
* Experience of working within a mental healthcare setting.
* Experience in providing clinical pharmacy services.
Skills
* Excellent communication and interpersonal skills, both written and verbal
* Advanced computer literacy skill.
* Excellent communication skills with demonstrable ability to present complex or sensitive information to clinicians and senior pharmacists.
