This work will be planned wherever possible. Salary: £25,750 - £27,795 per annum Reports to: Cwm Taf Young Persons Team Leader (Barod) and RCT Families Plus Team Manager Team Members Reporting to Job Holder: No direct reports. Contract: Permanent, in line with funding. THE SERVICE The Resilient Families Service delivers effective, timely and family focused early intervention services to families across RCT with the aims of building resilience. The Resilient Families Service is responsible for the delivery of effective Team Around the Family arrangements across the County Borough. As part of the Resilient Families Service, it is the responsibility of the Families Plus Team to deliver a short term package of support to families with complex and/or entrenched needs in order to build positive engagement and reduce levels of risk. The aim of Resilient Families Service substance use Team is to engage people who use drugs or alcohol, and or their concerned others, in thetreatment system and support them when accessing it.This includes early intervention, engaging and assessing service users, needle exchange, brief interventions, outreach, training, case management, health promotion and harm reduction initiatives and recovery and aftercare services. Barod services are provided in a non-judgmental, non-discriminatory, and professional manner and in a way that demonstrates courtesy and respect for individuals using substances, and sensitivity to their unique personal situations and experiences. ROLE PURPOSE To support and deliver interventions to families referred to the Resilient Families Team who havebeen identified as having substance use issues. As part of the Resilient Families Service, the Resilient Families Service Substance Use Worker will deliver a short term package of support to families with complex and/or entrenched needs in order to build positive engagement and reduce levels of risk. MAIN AIMS AND RESPONSIBILITIES AIMS To provide practical, reliable and flexible substance use support to families across RCT who have been identified as requiring intensive intervention as part of the Resilient Families Service as a result of their complex and/or entrenched needs. To work in partnership with all professionals and services involved in delivering aspects of Family Plans to ensure the effective delivery of outcomes for families. To deliver formal and/or informal substance use intervention to families in order to increase resilience and reduce risks associated with substance use as stipulated within Family Plans and as directed by the Families Plus Team Manager. To contribute to the ongoing development of the Resilient Families Service across RCT. Providing evidence-based substance use interventions and support to parents, carers, children, and young people who are impacted by parental substance Use, domestic abuse, mental health. To work collaboratively with other agencies within Rhondda Cynon Taf such as Cwm Taf Morgannwg UHB, CDAT, CMHT, CAMHS, Housing and BAROD to provide a holistic integrated family support service. RESPONSIBILITIES To deliver reliable and flexible substance use support to families with complex and/or entrenched needs as prescribed in the Families Plans compiled by the Brokerage and Review Officer in the Assessment, Brokerage and Review Team. To be responsible for an individual caseload, including demonstrating effective time management and prioritisation of workload To provide a central point of contact for families and professionals and be proactive in addressing issues that may lead to either parties disengaging from the process, liaising with the Engagement and Assessment Officer in the Assessment, Brokerage and Review Team as required To undertake direct work within a strengths based approach with families and extended family members both within and outside the home to address barriers to engagement including evening work as required To promote and facilitate families’ positive engagement with key universal and specialist services such as schools, education services, health services and housing services to ensure the sucessful delivery of the Family Plan. To support families to overcome barriers to change as identified in the Family Plan, supporting them to build their confidence and skills to do so To deliver substance use interventions within Tier 2 of the RCT Parenting Framework by supporting parents to implement parenting strategies within the home environment To identify any emerging family needs to the Families Plus Team Manager liasing with the Brokerage and Review Officer to determine opportunities and an appropriate course of action to address these needs within the existing Family Plan wherever possible Work closely with the Brokerage and Review Officer to review Family Plans with families and evaluate interventions as required feeding directly into the planning process To be prepared to have open and honest dialogue with families in relation to what is and what is not acceptable behaviour, providing clear expectations of what needs to be done to achieve improvements and the potential consequences for the family of not doing so. To transport children, young people and families when necessary, ensuring that the legal requirements for doing so are met (e.g. using appropriate baby car seats) To liaise with all professionals and services involved in delivering aspects of the plan to ensure day to day delivery is co-ordinated and supported, providing professional challenge as required to ensure progress and escalating issues for concern to the Brokerage and Review Officer in a timely manner To work in partnership with colleagues from across the Resilient Families Service as well as external organisations as part of a multi-agency support package to maximise the benefits for all families. Convene and where required chair meetings with professionals advocating for families and/or representing the Families Plus Team as required. To attend and contribute to Child Protection Strategy meetings for open case families requiring step up to statutory Children’s Services Teams with the support of the Team Manager. To undertake necessary recordings and maintain accurate, up to date records within the Resilient Families Service Management Information System in line with Divisional procedures and guidance. To bring to the attention of the Families Plus Team Manager any immediate concern regarding the saftey of a child, young person or family member, making referrals to MASH and Adult Services as and when required To ensure all child protection procedures are followed in accordance with the 'All Wales Child Protection Procedures.' and Cwm Taf Safeguarding Board procedures and guidelines To provide a quality, flexible service that promotes equality and celebrates diversity for children and their families, recognising cultural differences in families and making appropriate arrangements to meet needs (e.g. translator) To fully implement the Lone Working Policy, carrying out visual risk assessments to determine safety of the working environment; children, young people and families in that environment; ensuring own safety OTHER DUTIES Contribute to the monitoring and reviewing procedures for the service, including the submission of quarterly reports demonstrating performance against Key Performance Indicators. To participate in relevant training in line with the requirements of the Resilient Families Service and Barod services as agreed with line managers. To participate in appraisal and supervision proceses, including joint supervision with Resilient Families and Barod managers, within agreed timescales To participate team meetings and other staff support mechanisms deemed appropriate. Be prepared to undertake professional development training as required. Provide advice, support and training to colleagues in partnership agencies as necessary. Essential Desirable Qualifications and training Expected to meet basic skills in numeracy and Literacy. Health and Social Care qualification (or equivalent or relevant professional qualification) or willing to work towards. Trained and/or knowledge in the fields of parenting, emotional literacy, resilience, and solution focussed approaches, including evidence based programmes such as Parents as First Teachers, Webster Stratton, Take Three, Parenting Puzzle, ELSA and Thrive. Qualification in teaching, childcare, youth work, parenting or related fields Role-related knowledge, skills & experience Experience of working with young people and/or families Knowledge of substance use treatment and care pathways Knowledge of substance use including harm reduction advice, information and interventions A knowledge of the effects of deprivation on children, young people and families Experience of undertaking direct work with children, young people and/or their families. An understanding of the nature of substance use, its’ effects on individuals and significant others, with knowledge of the range of interventions and approaches available when working with service users, including diversionary activities Experience of delivering plans of work for groups or individuals based upon identification of need.
