Due to investment, an opportunity has arisen for TWO forward thinking, proactive and driven Pharmacy Technicians to join Home First, supporting patients following discharge to have their medicines optimised and reach an appropriate level of independence.
You should have excellent communication skills, work well in a team and be able to manage your own workload. In addition to working alongside our Home First Pharmacist, you will have the full support of our friendly and dynamic community pharmacy team.

As a key member of the multidisciplinary Home First team:
Gather information concerning patients' current medication/medical history
Provide in-reach to the hospital to support discharges to community
Carry out domiciliary visits to patients where appropriate
Provide assessment of patients’ ability to manage their own medicines
Produce MAR charts to support medicine administration by Home First care staff
Provide medicine-related advice to patients, families and carers
Liaise with other service providers to improve the transfer of care between care settings
Participate in MDT meetings and participate in service development
Carry out clinical audit collection.
The successful candidate must have access to a car and hold a valid driver’s license for work purposes. This post offers a fantastic opportunity to enhance your technical and leadership skills, with the support of an experienced senior pharmacist, whilst contributing to a vital service.
