The Ambulatory Wound Care service has an exciting opportunity for an enthusiastic, dynamic staff nurse to join our forward-thinking community service. You will be responsible for the assessment, delivery, and evaluation of nursing care for patients with a wide range of health needs and wound care in a clinical environment. Support will be given to develop existing skills and new skills.

You will be expected to support your peers, the team leader, and the organization in promoting the Trust's values and behaviors.
The hours of work are between 09:00 and 17:00, over 5 days. You will be expected to provide some weekend and Bank Holiday cover.
You will be required to drive and have access to a vehicle to cover the service as needed.
Main Duties of the Job
We are looking for a Band 5 staff nurse to support the Caseload holders in delivering high-quality, efficient, and safe care. We welcome newly qualified, community experienced, or those with no previous experience; for our teams, it is about the individual.
You will need to be able to work independently, possess good time management and organizational skills, and be innovative and flexible to work in other areas if required to meet service needs across the footprint of Central Cheshire Integrated Care Partnership.
We are looking for an enthusiastic person who possesses both clinical and digital skills to support the provision of quality care. You will work closely with specialist teams such as TVN, Podiatry, and Vascular. Our patients are varied and often complex.
Patient education is a vital part of our service. We operate with some patients a shared care approach to wound care supported by digital technology. This has enabled us to achieve a gold improvement matters award for this concept. Patient feedback has been very positive.

About Us
Mid Cheshire Hospitals NHS Foundation Trust (MCHFT) provides good quality, safe, and effective healthcare to the people of Cheshire and beyond. The Trust, which manages Leighton Hospital in Crewe, Victoria Infirmary in Northwich, and Elmhurst Intermediate Care Centre in Winsford, was established as an NHS Trust in April 1991 and became a Foundation Trust in April 2008.

Job Responsibilities
1. To work under the direction of the Team Leader and be responsible for the assessment and management of the health needs of an identified population and for the development, implementation, and evaluation of programmes of care within a range of target groups including patients with long-term conditions and complex nursing needs.
2. To support, and wherever possible maintain patients in their own preferred place of care, enabling them to maximize their independence and optimize their quality of life.
3. To be expected to deputize in the absence of the Case Managers.
4. To liaise with all healthcare professionals and other agencies to ensure appropriate and effective service delivery.
5. To work within the agreed framework for Community Nursing Services.
6. To assess, provide and review holistic nursing care, advice, and information to patients and carers within the clinic setting in a safe and professional manner using evidence-based knowledge.
7. To undertake initial assessments on receipt of specific referrals to the caseload, acknowledging the needs of clients and their carers.
8. To assess, plan, implement, and evaluate a plan of care to meet the patients' physical, social, emotional, and environmental needs, encompassing a rehabilitative/self-care approach where necessary and as per district nursing contract.
9. To provide skilled nursing care and ability to undertake clinical tasks including wound care, leg ulcer assessment, and management.
10. To be able to recognize, interpret, and analyze clinical data related to the individual clients' needs including client observations.
11. To demonstrate, teach, and supervise the nursing care that can be safely delegated to the patient/carer giving support as necessary.
12. To promote clinical excellence and an evidence-based approach to care by complying with the agreed Trust policies and procedures and maintaining clinical competencies in line with the competency framework.
13. To participate in the delivery of care involving complex needs and supporting carers and relatives in distressing circumstances.
14. To refer people to other practitioners when needs and risks are beyond one's own scope of practice or require longer-term support.
15. To prioritize and manage own caseload & client group.
16. To assume responsibility for the clinical caseload in the absence of the Team Leader/Senior Lead delegating work appropriately within the competency framework.
17. To maintain accurate personal and professional records adhering to MCHFT and Nursing and Midwifery Council policy and guidelines.
18. To ensure the most efficient and cost-effective utilization of all resources (human, physical, and financial).
19. To report accidents/incidents of patients or self which occur whilst on duty and to complete on the Incident reporting system immediately. To participate in any investigations as necessary.
20. To contribute to and cooperate in the coordination of off-duty and holiday leave with colleagues, ensuring service cover is maintained.
21. To maintain all aspects of MCHFT Integrated Governance Strategy and requirements including active participation in appraisal and clinical supervision in accordance with Trust policy.
22. Act at all times in a manner which illustrates respect for privacy, dignity, and confidentiality.

Salary: £29,970 to £36,483 per annum, pro rata.
