We have an exciting opportunity for an Advanced Clinical Practitioner (ACP) to work in our Integrated Frailty Crisis Pathway. This role will initially sit within our community based integrated service, rotating through our Single Point of Access and integrated Urgent Community Response/ Hospital at Home service. There will be scope to rotate through other aspects of the Acute Frailty Service including the ED in reach team, Frailty Same Day Emergency Care and inpatient Older Peoples Advice and Liaison. We welcome applications from both Allied Healthcare Professionals and those with a nursing background who meet essential criteria. Within the Royal Surrey Frailty services, we have expanded our ACP workforce to help deliver excellent care to all older people with Frailty. We have several established posts within the Acute Frailty Team (AFT), Frailty Same Day Emergency Care (FSDEC), Urgent Community Response (UCR) and Hospital at Home (H@H). These posts contribute to the weekend rota for H@H. With time we hope to be able to rotate trainee ACPs through several services to give them a broader training experience. You will be required to work as a semi-autonomous clinician within the service and will be supported in your training through our ACP programme aligned to the Frailty Academy. You will have a dedicated clinical supervision and will be supported in your professional development in order to fulfil your on-going commitment to the 4 pillars of Advanced Clinical Practice. We provide a 7-day service and it is likely you will have to contribute to weekend and evening rotas.
