We are looking for a highly motivated individual to join our Primary Care Team as a patient care coordinator.
As part of our experienced care coordination team, you will be working closely with a group of dynamic and friendly people. You will be mainly based at Swanswell Medical Centre, but you may be working across 1-3 different sites, with different people in multiple departments across the surgeries.
Care coordinating is a new, fulfilling role, working closely with patients, ensuring we provide them with quality care and the support that they require. You may be dealing with high needs patients regularly, and advocating for best practice.
You may also work closely with social prescribing link workers, clinical pharmacists, doctors, nurses, HCAs, admin staff and management teams to ensure patients are being directed to and receiving the most suitable care for them.
You may be given a caseload of identified patients and be required to ensure that their changing needs are addressed by taking into account local priorities, health inequalities and/or population health management risk stratification. You may also be given targets to ensure optimal patient care.
It is a self-motivated role, that offers room for a blend of independent and multidisciplinary work.
Main duties of the job
Your main duties will include:
* Helping people to manage their needs through answering queries, making and managing appointments and ensuring that people have good quality written or verbal information to help them make choices about their care, using tools to understand people's level of knowledge and confidence in skills to manage their own health.
* Supporting patients to utilise decision aids in preparation for a shared decision-making conversation.
* Providing coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches and other primary care professionals.
* Work with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients and, where required and as appropriate, refer people back to other health professionals within the PCN.
* Advocating for "best practice" throughout the surgery, ensuring our patients get the best care we can deliver.

* Access to own transport and ability to travel across the locality on a regular basis, including to visit people in their own home.
* Experience in Primary Care.
* Experience in working independently and as part of a wider team.
* Good knowledge of MS Office and Outlook.
