These are full-time/part-time locum for service (LAS) posts in Genitourinary Medicine with or without an on-call commitment. They are replacing vacancies at specialty registrar level in the Department and are therefore available immediately. These posts are fixed term until August 2025 in the first instance but may subsequently be extended pending national specialty training recruitment outcome. The Genitourinary Medicine service comprises the centralised HIV outpatient service based at Brotherton Wing Clinic, Leeds General Infirmary; and the integrated Leeds Sexual Health service whose hub clinic is based at Beeston Hill Health Centre, with other satellite clinic sites around the city. We have active SH/HIV research and teaching programmes. Leeds Sexual Health (LSH) provides integrated Contraception and Sexual Health services and third-sector provision as part of a consortium between LTH, Leeds Community Healthcare Trust, the Leeds GP confederation and Forum Central (voluntary sector). LSH provides sexual health, contraception and HIV prevention including complex HIV Prep with daily mixed clinics comprising same-day and booked appointments with self referral and triage for emergency cases, plus online and postal/e-prescription services. There is 6-day opening at the Beeston hub including evening access Monday and Thursday (no Saturday clinic expectation for resident Drs), plus further spoke sites operational around the city. There are, in addition, over 6000 HIV appointments per year. Leeds Local Authority is the highest prevalence area in Yorkshire & Humber having exceeded HIV population prevalence over 2/1000 since 2012, and as such the department has built up a reputation as a centre of expertise for HIV/AIDS. Leeds committed to Fast Track City status in 2023, working towards the HIV zero goals of 2030 and restarting NHSE ED opt-out BBV testing in 2024. LTH is the West Yorkshire HIV network regional centre with a local HIV cohort of ~2500 including spoke clinic sites in Dewsbury and Wakefield. Most of these patients are outpatients but there is a significant in-patient workload which is managed on Ward J20 at St Jamess Hospital, led by the ID consultant team, with registrar and junior support from a combination of ID & GUM teams. We work very closely with HIV support services like BHASkyline & Yorkshire Mesmac, with engagement worker & peer mentors supporting retention in care. MEDICAL STAFF IN THE DEPARTMENT The LTHT GUM consultant team: Dr Amy Evans (Lead clinician), Dr Sarah Schoeman (HIV lead), Dr Anna Hartley (Medical Co-lead, Leeds Sexual Health) Dr Harriet Wallace, Dr Janet Wilson (Research leads), Dr Emma Page (Virology), Dr Natasha Astill; HIV Consultant colleagues Dr Amy Tobin and Dr Vinu Nair & joint GUM-GIM Consultants Dr Khine Phyu and Dr Mike Ewens. Other medical staff currently comprise 4 NTNs (StRs), 1 Speciality Doctor, 1 IMT trainee,1 FY1/FY3 and 2 Innovative GP trainees. Designated staff Within LSH we work as an integrated team with a further 2 SRH Consultants, and sessional/SAS grade Drs; plus 2 SRH StRs. There is a skilled and designated health advisor, nursing and HCA establishment with advanced nursing practice, research nurses and nurse prescribing in place. In BWC HIV service we work with Infectious Diseases Consultants & StR/research staff. There are HIV pharmacists & technicians, an HIV Clinical Nurse Specialist team and Health advisers/BBV nurses who maintain continuity across HIV and SH services, and support our ED BBV testing programme. KNOWLEDGE, SKILLS AND EXPERIENCE REQUIRED Given the complex care requirement +/- oncall commitment at registrar equivalent level, the appointee must hold a basic medical qualification ie MBBS/MBChB or equivalent and full MRCP with PACES (or equivalent). DFSRH/ specialty diplomas +/- procedural letters of competence in contraception are desirable. Experience The appointee must be able to demonstrate clinical experience in the management of acute unselected medical cases, hence usually 4 years of post-registration medical experience. Training in the management of sexually transmitted infections and HIV will be offered during induction to the post. Gynaecology/reproductive health experience is also desirable DUTIES OF THE POST After induction, daily outpatient clinic sessions in GU Medicine/integrated Sexual Health across our clinic sites and after further training in outpatient HIV management. Further clinical activities include virtual ward rounds, MDT, clinical administration, Discharge letters and GP letters/referrals and prescriptions. Urgent clinic patients must be assessed, and HIV admissions arranged and assessed. Ward working patterns are in evolution and may involve a block attachment to the J20 ward eg for 2-4 months, with combined in-patient J20 / GUM clinic working. If there is oncall slot capacity, on call commitment is with fellow Specialty registrars in GU medicine & Infectious Diseases/Combined Infection training on the J20 current joint rota, which is 1 in 7-8 on average. You would be expected to do an evening GUM clinic 1 week in 3, until 8pm as part of your oncall banding. LEVELS OF RESPONSIBILITY The post-holder is responsible to the Lead clinician and an Educational supervisor will oversee their educational supervision, and the GUM, SRH and ID consultants will contribute to clinical supervision along with our skilled nursing staff & SAS grade Drs. TEACHING AND RESEARCH/STUDY AND TRAINING The non clinical sessions will involve audit, teaching and CPD. It is expected that there will be involvement in the teaching of medical students and other support staff, when required. Involvement in the research activities of the GUM department study portfolio and HIV service through CLRN-supported studies is actively encouraged.
