An exciting full time/part time opportunity has arisen to recruit Registered Nurses to work at Darlington Memorial Hospital or University Hospital of North Durham within the Discharge Lounge. We strive to provide an effective and efficient service to our patients who require our input in assisting with their discharge home. The Discharge Lounge provides a warm caring environment in which patients can safely wait for discharge home or transfer to an alternative setting such as community hospital within the Trust.
Excellent communication skills are essential to ensure a safe discharge and an increase in patient flow around the hospital. The postholder will be required to communicate with the Wards, Doctors, Pharmacy, Relatives, Carers, and Transport and Patient Flow Team. The postholder will supervise discharge lounge Health Care Assistants, liaise with the Discharge management team/ward coordinators and relatives. The postholder will be responsible for patient care during their time in the discharge lounge and efficient coordination of booking patient transport and checking prescribed medication in readiness for safe discharge.
