Job Summary
An exciting opportunity has arisen for an enthusiastic Community Practitioner to join the Kolvin Service. The Kolvin Service comprises a regional community adolescent forensic mental health service, a nationally commissioned FCAMHS, and mental health and forensic services into two Secure Children's Homes (SCHs) in the North East for young people aged 10-18 years.
The Kolvin Service collaboratively leads on the implementation of the SECURE STAIRS trauma-informed framework at Aycliffe Secure Centre and Kyloe House SCHs. The SECURE STAIRS framework represents a shift in focus within the secure estate from interventions focused solely on young people to interventions focused on the whole system.
The challenging nature of this post requires suitable applicants to be experienced mental health or learning disability nurses who are highly motivated, enthusiastic, well-organized, and possess excellent communication skills, sound clinical assessment, risk management, and intervention skills. A commitment to multi-disciplinary working is essential. The successful candidate will have the opportunity to learn and develop skills delivering therapeutic work and trauma-informed care as well as demonstrate the capacity to understand the impact of working with trauma on professionals and carers.
Main Duties of the Job
The post holder will:
1. Assist the MDT in service delivery.
2. Be responsible for comprehensive mental health assessments, including risk assessments, management plans, and care plans.
3. Support coordination of therapeutic interventions with young people.
4. Provide a service for children and young people with complex trauma, who may present a high risk to themselves, high risk to others, or are highly vulnerable.
5. Offer assessments, carry out interventions, and support professionals and carers working with children and young people to uphold the principles of the SECURE STAIRS Framework.
6. Consider carer needs and ensure carer and family involvement in care planning where appropriate.
7. Deliver evidence-based clinical and psychologically based interventions.
8. Provide compassionate care based on empathy, kindness, respect, and dignity.
9. Incorporate a client-centered philosophy into nursing practice and collaborative care.

Job Description
Job Responsibilities
1. Undertake a comprehensive Core Assessment of Service Users' health and social care needs, including clustering in accordance with Care Coordination Policy.
2. Develop a Clinical Formulation of needs to inform Care Planning and ongoing delivery of care.
3. Assess and manage risks to service users and others, who present with complex and significant risks, involving possible harm to self or others, and formulate appropriate interventions and risk/safety planning.
4. Undertake Care Planning, Implementation, and Review of Service Users presenting with enhanced needs in accordance with the Trust's Care Coordination Policy.
5. Demonstrate Clinical Reasoning when selecting appropriate treatments/psychological based interventions.
6. Make a judgment on whether clients have the capacity to give informed consent and seek additional advice when necessary.
7. Reflect and critically appraise own performance, knowing when to seek advice and support from more experienced clinicians.
8. Advocate on behalf of Service Users and Carers to ensure that Trust and Local Authority statutory responsibilities are met.
9. Exercise clinical and professional judgment, which may conflict with other professions or the service user.
10. Understand and apply policies and procedures to ensure that Trust and Local Authority statutory responsibilities are met.
Person Specification
Education and Qualifications Essential
* Professional Qualification and current Professional Registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker, or Clinical Psychologist.
* Mentor Training.
* Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post.
* Proven track record of post-registration clinical experience relevant to the post.
* Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults), and Child Protection.
* Up-to-date knowledge/experience of relevant legislation.
Experience and Knowledge Essential
* 12 to 24 months of band 5 qualified nurse experience, dependent upon service need.
* Awareness and understanding of professional accountability, NMC standards, and other local and national initiatives.
Skills and Competencies Essential
* Proven ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs.
* Risk Assessment and Risk Management/Safety Planning skills.
* Demonstrate an understanding of effective and appropriate treatment options, including a range of evidence-based clinical approaches as identified in N.I.C.E. Guidance.
* Demonstrate effective written and verbal communication skills.
* Evidence of a high level of professional, organizational, and leadership skills (self and others).
* Ability to adapt within a changing environment.
