Are you or have you previously worked as a Support Coordinator in the health or social care sector?
We are looking for an experienced Coordinator predominantly with reablement and extra care experience, who is passionate about delivering person centred support with a dynamic and innovative approach to join our team.
About the Role:
The postholder will support the Registered Manager in the operational management and leadership of the Councils CQC regulated service Borrowdale House, reablement and extra care service. By ensuring the performance, quality assurance and continuous improvement of the service and contribute to the Council’s aims and objectives within an overall framework that safeguards adults and promotes the health, independence and wellbeing of people who use services.
As Coordinator you will support the Registered Manager in all aspects of service delivery including workforce management and quality assurance to ensure the provision is of high quality CQC registered care and support, that promotes independence and well-being for people.
Working with the multidisciplinary team based within Borrowdale House, people using our services and their families, you will work in collaboration with our health colleagues to ensure people receive a quality service that adheres to all CQC requirements and South Tyneside Council’s vision and strategy.

About You:
You will have experience in Coordination of a regulated service, with experience or good understanding of reablement, extra care and/or health services.
You will be passionate in the delivery of high-quality services, enabling people to live better lives. You will be accustomed to supporting diverse teams through embedding systems and processes enabling our services to remain effective, responsive, safe, well-led and caring.
