Raising the bar for people-centered outreach services, you’ll enable individuals to learn skills and knowledge to understand and manage their own situations more independently. No matter how complex someone’s mental health needs, you’ll provide support that helps decrease isolation, promote community integration, and help individuals seize new opportunities.
The role involves rotational shifts, including evenings, weekends, and bank holidays. We are open to discussing flexible working arrangements to promote work/life balance.
Role Responsibilities
You will support clients in the community across Manchester, encouraging engagement in drop-in activities at community centers, attending medical appointments, and supporting social inclusion activities. You will work with clients in their homes, assisting with medication management, activities of daily living, and providing emotional support, reporting concerns to the Outreach or Nurse Manager.
Working alongside qualified nurses, you’ll recognize signs of deteriorating mental health and help implement interventions. Promoting independence, well-being, and healthy lifestyle choices, you’ll participate in activities such as walks, theater trips, gym sessions, and social networking to support clients’ community integration.
The Ideal Candidate
We are seeking an Outreach Support Worker with experience working with individuals with long-term mental health issues and a solid understanding of their challenges and recovery processes.
Great team players with energy, strong communication skills, flexibility, and a positive attitude are essential to meet targets in a dynamic environment. Empathy, patience, and a passion for helping others are also crucial. A driving license is highly desirable.
