The Licensed Practical Nurse (LPN) helps achieve our mission by delivering age appropriate care to the patient in the home setting utilizing standard nursing best practice: implementation of plan of care as established by a Registered Nurse, and within scope of practice defined by the NH Board of Nursing. LPNs will effectively interact with patients, caregivers and other healthcare team members while maintaining all standards of professional nursing. LPNs are supervised by the Registered Nurse assigned to the patient.
Essential Job Functions
1. Follows appropriate plan of care (POC) developed by the Registered Nurse in partnership with the patient, patient representative (if any) and/or caregiver(s) based on thorough assessment incorporating all aspects of patient limitations and goals.
2. Documents accurate and ongoing treatments: nursing interventions, patient response to care provided, patient/caregiver educational needs, problems, capabilities, limitations and progress toward goals. Documentation includes evidence of appropriate patient/caregiver teaching, and the understanding of these instructions in accordance with agency guidelines.
3. Engages the patient/caregiver in meaningful education as indicated per care plan.
4. Communicates with all physicians involved in the POC and other health practitioners related to the current POC.
5. Demonstrates responsibility to effectively communicate the patient’s achievement toward the goals within the POC with members of the interdisciplinary Team and other health professionals through team/patient care conference.
6. Participates in interdisciplinary team, agency projects, committees, QAPI program and/or other activities upon request and approval from direct supervisor.
7. Attends Agency In-service programs.

Requirements
* Graduate of an accredited school of nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC).
* One year of recent general medical/surgical or skilled/acute care nursing experience. If hired, be available Monday-Friday for the first weeks of employment to complete the required training.
