The Clinical Review Team, review patients with 117 after care packages on behalf of the ICB. The team will review patients with learning disabilities, mental health and acquired brain injuries, so all level of registrations welcome.
The aim of the review is to ensure the funded package of care meets the needs of the patient, its safe and ensures financial efficiency.
This work covers all parts of the patient journey from transition from hospital into the community, where the patient will live with a funded package of care in the least restrictive environment.
The role will a require presentation of cases to the Quality Assurance Funding panel.
The Clinical Review Practitioner will hold a caseload of Patients with Section 117 after care needs who are in funded packages of care.
The review will be in collaboration with the provider, the patient, and members of the MDT. The review will ensure the placement is ensuring financial efficiency and the patient is safe and is living in the most least restrictive environment. The review will also ensure the patient has the right legal framework in place to support their care delivery. The role will also include to lead on applying to Court for application and renewal of DOLS.
Work alongside colleagues from contracting, performance and information to achieve the commissioner targets
Communicate in a way that recognises difference and ensures that people feel included and their individual communication needs are met
This will involve completion of the Reviewing document and potentially other clinical tools for example from the risk management or safeguarding toolkits.
Part of this would include presenting at the Quality Assurance panel to seek permission for funding for new placements, this would be achieved by completing the Joint Health Assessment and Social Care (JHAS)

To undertake a programme of S117 reviews in collaboration with other members of multi-professional teams where required. This will involve completion of the Reviewing document and potentially other clinical tools for example from the risk management or safeguarding toolkits. Placement reviews will be required, providing evidence that safety and quality issues are addressed and that interventions are recovery focussed.
To present the assessment to the S117 Quality Assurance Forum for new packages of care, and cases they require split funding. changes to a package of care such as increase in one-to-one observations.
To provide support to the service user and carer(s) through the review process, involving them in choices and decision making as appropriate, and keeping them informed at all stages. Referral to and liaison with appropriate Advocacy and legal representation services may be necessary.
To collaborate and liaise with current and potential placement providers and representatives of mental health and social care services in Coventry and Warwickshire and outside of the Trust’s geographical area where necessary.
To identify any unmet need or blockages or gaps in the development of robust plans and highlight them to the team manager.
