The post holder will be based within the Care Home Liaison Service, initially based at Stirling Community Hospital. This post is for 37hrs. The Care Home Mental Health Liaison Service supports care homes across Forth Valley was developed in response to an identified need for ongoing clinical support and education for all care home staff with regard to mental ill health in older people. This is a nurse-led service, supported by a part time Consultant Psychiatrist. It provides a responsive and comprehensive mental health assessment at times of crisis/need to any older person within a care home setting, and those with a cognitive impairment (regardless of formal diagnosis). The suitable candidate should have experience in working within an older adult mental health setting and possess in depth knowledge of common conditions, assessment tools and treatments for older people. Excellent communication skills, time management and an ability to work autonomously is essential. The post involves close partnership working with health and social care professionals in care home settings to meet the mental health needs of the older people and promote person centred care maintaining close links with various Forth Valley Community Mental Health Teams for Older Adult. You should be a registered mental health nurse educated to degree level. Post graduate qualifications relevant to this area, or willing to work towards is desirable. Ability and willingness to travel throughout the area of service delivery is essential for this post.
