An exciting opportunity has arisen for a Band 6 Community Practitioner to join our South Central Team within the Older Adults Services in Northumberland.
Our Older Adult Services provide specialist mental health assessment and intervention for older persons with complex and enduring mental health problems, including those younger persons with an ongoing dementia type illness. The multi-disciplinary team comprises a highly skilled and experienced workforce of Consultant Psychiatrists, Nurses, Psychologists, Occupational Therapists, and Physiotherapists.
Main duties of the job
We deliver a wide range of evidence-based interventions through clearly defined care pathways. We have developed a comprehensive supervision structure for staff including individual supervision and peer support through our clinical networks linked to our care pathways. We believe in the development of all our staff and encourage personal development.
Our Community Practitioners are expected to undertake assessments and implement evidence-based treatment packages. They act as Care Coordinators for complex cases involving multi-disciplinary working. They will be responsible for the supervision of students and junior staff. They will also do duty on a rota basis.

Job responsibilities
We deliver a number of services including assessment and treatment of complex mental health issues, assessment of memory, and post-diagnostic groups. We also provide a comprehensive behaviour support service.
Our mental health teams have a trauma-informed approach that acknowledges the person's life experiences in relation to their mental health. This approach encompasses the principles of safety, choice, trust, working collaboratively, and empowerment.
We are seeking to appoint RMNs/AHPs with a wide range of experience working with mental health clients. The successful candidate will be innovative, forward-thinking, diverse, and able to work autonomously. They will need excellent communication skills, both written and verbal, with a good knowledge of different mental health interventions.
Person Specification
Education and Qualification
* Professional Qualification and current Professional Registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker, or Clinical Psychologist.
* Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post.
* Proven track record of post-registration clinical experience relevant to the post.
* Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults), and Child Protection.
* Up-to-date knowledge/experience of relevant legislation.
* Post Registration Qualifications in Psychological Interventions.
Knowledge and Experience
* 12 to 24 month band 5 qualified nurse experience dependent upon service need.
* Awareness and understanding of professional accountability, NMC standards, and other local and national initiatives.
Skills and Competencies
* Proven ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs.
* Risk Assessment and Risk Management/Safety Planning skills.
* Demonstrate an understanding of effective & appropriate treatment options including a range of evidence-based clinical approaches as identified in N.I.C.E. Guidance.
* Demonstrate effective written and verbal communication skills.
* Evidence of a high level of professional, organisational, and leadership skills (self & others).
* Ability to adapt within a changing environment.
* Ability to work under pressure.
* Experience of working in a community setting.
