As a member of the clinical team, your expertise in medicines management and pharmaceutical care will help to optimise patient care and ensure the safe, effective use of medications.
As an experienced pharmacist with a solid background in primary care, you will contribute to the clinical team’s efforts, supporting the safe and efficient use of medicines, repeat prescriptions, and pharmaceutical care.
You will also play a critical role in clinical decision-making, prescribing support, and enhancing treatment plans.
Responsibilities: Patient Care: Provide remote and in-person support to registered patients on medicines, repeat prescriptions, and other pharmaceutical queries.
Prescribing Support: Assist with audits, prescribing, and managing high-risk drugs to maintain patient safety.
Patient Assessment: Tailor advice to individual patients, considering their health, knowledge, and socio-economic context.
Prescription Management: Ensure accurate and timely re-prescribing using summary care records.
Clinical Risk Management: Identify clinical risks and provide solutions to ensure optimal patient care.
Protection Protocols: Respond to child and adult protection concerns and follow established protocols.
Collaborative Work: Collaborate with a multidisciplinary team (MDT) to support service delivery and contribute to quality improvement.
Clinical System Documentation: Record accurate information within clinical systems.
Regulatory Adherence: Follow the ethical standards and performance criteria set by the General Pharmaceutical Council (GPhC).
Professional Development: Participate in continuous professional development to support both personal and organisational growth.
Required Skills: Registered with the General Pharmaceutical Council (GPhC).
Completed or nearing completion of the CPPE Pathway.
Non-Medical Prescribing qualification.
Extensive experience working in a primary care setting.
Proven ability to manage care through remote and face-to-face consultations.
Strong clinical expertise in medicines management and repeat prescription management.
Excellent critical thinking, communication, and time management skills.
Familiarity with child and adult protection protocols.
Competence in using clinical systems and maintaining accurate records.
