We are recruiting for a full time Band 5 CPN to join our team, working within the Denbighshire Older Persons Community Mental Health Team. The post will be based in Denbigh, although some working in Rhyl will be required.
Main duties of the job
As a Care Coordinator, you will hold a caseload of patients with organic and functional mental health needs, travelling to see them in their own homes, in care homes and in-patient settings. You will also attend ward rounds and support the Care Homes CPN as needed. You will provide support to the Band 6 Duty Nurse on the Duty roster and ensure that Care and Treatment Plans are completed and adhered to, in line with the Mental Health (Wales) Measure 2010.
