The post holder will work within a small care team to deliver community-based palliative nursing care for children and young people with life-threatening and life-limiting illnesses. This role includes providing skilled holistic nursing care in the family home or other requested environments, including hospital-based care. The post holder will offer emotional support and advice to children, young people, and their parent/carers, and will be responsible for planning, implementing, and evaluating individualized care during each respite visit in the absence of the parents or carers. Additionally, the post holder will collaborate with the end-of-life team leader to plan and deliver end-of-life care to children on the Jessie May caseload. Availability for on-call overnight and weekend shifts on a rota basis may be required.
The post holder will manage a caseload, primarily conducting visits in North Somerset, but must be mobile across the entire geographical area that Jessie May covers.
Main Duties of the Job
Working as part of the Jessie May care team, your main duties will be:
* Deliver high standards of individualized holistic care and support to children and young people with life-limiting conditions and their families.
* Supervise the child or young person in the absence of their parents or carers.
* Communicate effectively with all members of the Jessie May Care Team.
* Promote evidence-based practices beneficial to the care of children on the Jessie May caseload, in accordance with clinical governance guidelines.
* Undertake all appropriate aspects of nursing care required within the home setting and develop additional clinical skills as needed.
* Manage own workload during respite visits and office-based duties.
* Work within UHBW policy and Jessie May Guidelines.
* Facilitate effective communication with Jessie May colleagues and partner agencies.
* Collaborate with the line manager to improve services and enhance the quality of care for children and young people on the Jessie May caseload.
* Ensure compliance with Health and Safety requirements and report all accidents and incidents to the line manager.

Job Responsibilities

* Ensure adherence to UHBW policy and Jessie May Guidelines.
* Work with the end-of-life team leader and care team to deliver end-of-life care.
* Provide support and supervision to students and other staff as appropriate.
* Participate in implementing government directives.
* Follow emergency procedures for the child or young person receiving care.
* Administer medications prescribed by qualified medical practitioners.
* Assist with nursing needs as per the individualized care plan, encouraging independence.
* Listen to the requests of children and parents/carers, providing care consistent with their wishes.
* Offer emotional support using empathetic communication.
* Maintain written records of care provided in accordance with UHBW policy and Jessie May guidelines.
* Identify needs for additional emotional support and communicate these to the care team.
* Recognize the need for specialized bereavement support and maintain communication with the care team and Bereavement lead.
* Participate in service evaluation.
Person Specification
Qualifications and Training
* RSCN/RN (Child) or equivalent.
* Current NMC registration.
* Work towards relevant university accredited post-basic study programs or modules (e.g., teaching and assessing course/palliative care module).
Skills and Abilities
* Advanced communication skills, including experience of supporting difficult conversations.
* Ability to advocate for and empower vulnerable children and families.
* Leadership skills to respond to deteriorating patients effectively.
* Flexibility and adaptability in challenging environments.
* Effective time management and workload prioritization.
* Driver's license and ability to work remotely, including computer skills.
* Ability to manage and make clinical decisions for allocated caseloads.
* Maintain confidentiality and comply with GDPR guidelines.
* Serve as a role model for Jessie May and UHBW, including supervising students.
Knowledge and Experience
* Understanding of family-centered care and challenges faced by families on a palliative care pathway.
* Relevant post-registration experience, including palliative and/or end-of-life care.
* Evidence of teamwork and effective communication with multidisciplinary teams.
* Recognize own limits and seek appropriate support.
* Knowledge of safeguarding and timely action to minimize risk.
* Experience of lone working in community settings with children and families.
* Interest in working with children with severe, complex, or special needs.
Public Sector Language Competency
* Fluent English speaker to an appropriate standard.
